In general, plastic surgery is divided into two categories: cosmetic and reconstructive. Cosmetic surgery, generally sought by patients to improve their appearance, usually is not covered. Reconstructive surgery, to repair damage from disease, surgery, injury or some problem present from birth, often is. Some procedures fall into a gray area and might or might not be.
